The final days of the 20th century are uniquely symbolic. They provide an opportunity to consider the continuing challenges in surgical oncology in a much broader context. Science and technology have come of age in the past century and they provide the tools with which we can interpret and modify the natural world to shape the future. In this commentary, we consider these challenges and opportunities in the broader economic and social context from which progress in medicine is ultimately inseparable.
